5-FU Chemotherapy: The Good, the Bad, and the Ugly. What Every Cancer Patient Should Know About Fluorouracil Before Starting Treatment.  Learn the benefits, common side effects, serious toxicities, and evidence-based ways to reduce complications from 5-fluorouracil (5-FU) chemotherapy.

What is 5-FU?

5-Fluorouracil (5-FU) is one of the oldest—and still one of the most effective—chemotherapy drugs used today. Developed in the 1950s, it remains a cornerstone of treatment for many solid tumors because it is inexpensive, well studied, and highly effective when used appropriately.

Today, 5-FU is commonly included in chemotherapy regimens such as:

  • FOLFOX (5-FU + leucovorin + oxaliplatin)
  • FOLFIRI (5-FU + leucovorin + irinotecan)
  • FOLFIRINOX
  • Various chemoradiation protocols for gastrointestinal cancers
  • Head and neck cancer treatment
  • Anal cancer treatment

It is most frequently used for:

  • Colorectal cancer
  • Rectal cancer
  • Pancreatic cancer
  • Gastric cancer
  • Esophageal cancer
  • Head and neck cancers
  • Anal cancer

Like virtually every chemotherapy drug, however, 5-FU has two faces. It saves lives—but it can also cause significant toxicity.


The Good: Why Oncologists Continue to Use 5-FU

There is a reason this drug has survived nearly seventy years.

1. Proven survival benefit

Thousands of clinical trials have demonstrated that 5-FU improves survival in colorectal cancer and several other gastrointestinal cancers.

Continuous infusion regimens often produce higher response rates and slightly longer survival than bolus administration.


2. It combines well with other therapies

5-FU works synergistically with:

  • Oxaliplatin
  • Irinotecan
  • Radiation therapy
  • Leucovorin (folinic acid)

Leucovorin does not reduce toxicity.

Instead, it enhances the effectiveness of 5-FU by stabilizing its binding to thymidylate synthase, increasing cancer cell death.


3. Decades of experience

Unlike many newer drugs, physicians understand:

  • expected side effects
  • optimal dosing
  • supportive care
  • dose adjustments

This long clinical experience helps make treatment safer than it once was.


The Bad: Common Side Effects

Most patients experience some degree of toxicity.

Fortunately, many side effects can be managed.

Fatigue

One of the most common complaints.

Causes include:

  • inflammation
  • anemia
  • poor nutrition
  • sleep disruption
  • dehydration

Helpful supportive therapies include:

  • walking
  • resistance exercise
  • adequate protein
  • sleep optimization
  • hydration

Mouth sores (Oral mucositis)

Painful ulcers may interfere with eating and drinking.

Evidence-supported approaches include:

  • salt/baking soda rinses
  • meticulous oral hygiene
  • avoiding alcohol-containing mouthwash
  • adequate hydration

Mucositis remains one of the classic toxicities of 5-FU.


Diarrhea

This is one of the most common dose-limiting toxicities.

Persistent diarrhea can lead to:

  • dehydration
  • electrolyte disorders
  • kidney injury
  • hospitalization

Patients should report diarrhea early rather than attempting to “tough it out.”


Low blood counts

Although less severe with continuous infusion than bolus dosing, 5-FU may reduce:

  • white blood cells
  • red blood cells
  • platelets

Continuous infusion generally causes less hematologic toxicity than bolus therapy.


Hand-Foot Syndrome

Especially common with prolonged infusions.

Symptoms include:

  • redness
  • burning
  • peeling
  • cracked skin
  • painful walking

Preventive measures:

  • moisturizing creams
  • avoiding friction
  • avoiding excessive heat
  • reporting symptoms early

Nausea

Usually mild compared with many chemotherapy drugs.

Most patients improve with modern anti-nausea medications.


Hair loss

Unlike many chemotherapies:

Complete hair loss is uncommon with 5-FU alone.

Many patients experience only mild thinning.


The Ugly: Rare but Serious Complications

These toxicities deserve immediate medical attention.


1. Cardiotoxicity

5-FU can injure the heart.

Possible complications include:

  • chest pain
  • coronary vasospasm
  • arrhythmias
  • heart attack
  • heart failure

Risk is higher in patients with:

  • coronary artery disease
  • prior heart attack
  • vascular disease

Chest pain during infusion should never be ignored.


2. Severe diarrhea and dehydration

This can become life-threatening.

Consequences include:

  • acute kidney injury
  • electrolyte imbalance
  • confusion
  • hospitalization

Prompt IV fluids often prevent serious complications.


3. DPD (DPYD) deficiency

Perhaps the most important complication every patient should know about.

Some individuals inherit reduced activity of the enzyme dihydropyrimidine dehydrogenase (DPD), which normally breaks down 5-FU.

Without enough DPD, the drug accumulates to dangerous levels, causing profound toxicity after even standard doses.

Symptoms may include:

  • uncontrollable diarrhea
  • severe mouth sores
  • low blood counts
  • confusion
  • seizures
  • multi-organ failure

Although uncommon, this toxicity can be life-threatening. Increasing numbers of experts recommend DPYD genetic testing or other approaches to identify patients at higher risk before treatment. Pharmacokinetic monitoring and individualized dosing may also improve safety in selected patients.

Many cancer centers now recommend pretreatment DPYD testing (genotyping, phenotyping, or both) before starting 5-FU or capecitabine. Patients with partial deficiency usually receive a substantially reduced starting dose, while those with complete deficiency are generally treated with an alternative chemotherapy regimen.

About 3–8% of people have an inherited partial deficiency of the enzyme that metabolizes 5-FU (DPD), and 0.1–0.2% have complete deficiency. Without testing, these patients can experience life-threatening toxicity from standard doses of 5-FU or capecitabine. Ask your oncologist whether DPYD testing should be performed before treatment.


4. Neurotoxicity

Rarely patients develop:

  • confusion
  • encephalopathy
  • imbalance
  • slurred speech

These symptoms require immediate evaluation.


5. Infection

Low white blood counts increase infection risk.

Call your oncology team immediately for:

  • fever over 100.4°F (38°C)
  • shaking chills
  • shortness of breath
  • uncontrolled vomiting

Who Is Most Likely to Experience Severe Toxicity?

Research suggests increased risk among:

  • older adults
  • patients with impaired kidney or liver function
  • patients with DPD deficiency
  • those with poor nutritional status
  • individuals receiving multiple chemotherapy agents
  • patients with significant cardiovascular disease

Older age and female sex have been associated with a higher likelihood of severe toxicity in some studies, underscoring the importance of individualized monitoring.


Can Side Effects Be Reduced?

While no therapy eliminates toxicity completely, several evidence-based strategies may help.

Conventional supportive care

  • Early dose adjustments
  • Antidiarrheal medications
  • Antiemetics
  • IV hydration
  • Mouth care
  • Blood count monitoring

Integrative therapies supported by evidence

Depending on the specific side effect, research supports consideration of:

  • Regular exercise for fatigue
  • High-protein nutrition during treatment
  • Maintaining hydration
  • Monitoring electrolyte status
  • Gut microbiome support after discussing probiotics or fermented foods with the oncology team
  • Stress reduction techniques
  • Sleep optimization

Always discuss supplements with your oncologist, as some may interact with chemotherapy.

  • Why am I receiving 5-FU instead of another chemotherapy?
  • Am I receiving bolus or continuous infusion?
  • What side effects should I expect?
  • Should I be tested for DPYD deficiency before treatment?

Final Thoughts

For many cancers, 5-FU remains one of the most effective chemotherapy drugs available. Countless patients have achieved long-term remission or cure because of fluorouracil-based treatment.

At the same time, 5-FU is not a benign drug. Fatigue, diarrhea, mouth sores, and hand-foot syndrome are common, while rare complications such as cardiotoxicity or severe toxicity due to DPD deficiency can be life-threatening if not recognized promptly.

The goal is not to fear chemotherapy—but to understand it. Patients who know what to expect, report side effects early, and combine excellent supportive care with healthy nutrition and lifestyle practices are often able to complete treatment more safely and successfully.
