5-Fluorouracil (5-FU) is one of the oldest—and still one of the most effective—chemotherapy drugs used today. Developed in the 1950s, it remains a cornerstone of treatment for many solid tumors because it is inexpensive, well studied, and highly effective when used appropriately.

Some individuals inherit reduced activity of the enzyme dihydropyrimidine dehydrogenase (DPD), which normally breaks down 5-FU.
Without enough DPD, the drug accumulates to dangerous levels, causing profound toxicity after even standard doses.

Although uncommon, this toxicity can be life-threatening. Increasing numbers of experts recommend DPYD genetic testing or other approaches to identify patients at higher risk before treatment. Pharmacokinetic monitoring and individualized dosing may also improve safety in selected patients.
Many cancer centers now recommend pretreatment DPYD testing (genotyping, phenotyping, or both) before starting 5-FU or capecitabine. Patients with partial deficiency usually receive a substantially reduced starting dose, while those with complete deficiency are generally treated with an alternative chemotherapy regimen.
About 3–8% of people have an inherited partial deficiency of the enzyme that metabolizes 5-FU (DPD), and 0.1–0.2% have complete deficiency. Without testing, these patients can experience life-threatening toxicity from standard doses of 5-FU or capecitabine. Ask your oncologist whether DPYD testing should be performed before treatment.

For many cancers, 5-FU remains one of the most effective chemotherapy drugs available. Countless patients have achieved long-term remission or cure because of fluorouracil-based treatment.
At the same time, 5-FU is not a benign drug. Fatigue, diarrhea, mouth sores, and hand-foot syndrome are common, while rare complications such as cardiotoxicity or severe toxicity due to DPD deficiency can be life-threatening if not recognized promptly.
The goal is not to fear chemotherapy—but to understand it. Patients who know what to expect, report side effects early, and combine excellent supportive care with healthy nutrition and lifestyle practices are often able to complete treatment more safely and successfully.
