Fatigue is one of the most common side effects of chemotherapy. While many patients assume fatigue is simply part of cancer treatment, chemotherapy-induced anemia (CIA) is often a major contributor—and one that can frequently be treated.

If you’re receiving chemotherapy and find yourself exhausted, short of breath, dizzy, or unable to perform normal daily activities, ask your oncologist whether anemia is contributing to your symptoms.

The good news is that both conventional oncology and evidence-based complementary therapies may help improve anemia, reduce symptoms, and improve quality of life.

I am a long-term survivor of an incurable blood cancer called multiple myeloma. My research and experience with evidence-based non-conventional therapies are the reasons why I have lived in complete remission from my incurable blood cancer since achieving complete remission in early 1999. I have learned that the best way to manage aggressive cancers is to combine the best of conventional and evidence-based non-conventional therapies.

I have come to believe that therapy-induced side effects can be life-threatening while ruining quality of life. Consider therapies shown to reduce possible side effects.

What Is Chemotherapy-Induced Anemia?

Chemotherapy damages rapidly dividing cells. Unfortunately, this includes the bone marrow cells responsible for producing red blood cells.

As red blood cell production falls:

  • Hemoglobin decreases
  • Oxygen delivery to tissues declines
  • Muscles and the brain receive less oxygen
  • Fatigue increases dramatically

Many patients develop anemia gradually over several months of treatment.

Common symptoms include:

  • Extreme fatigue
  • Weakness
  • Shortness of breath
  • Rapid heartbeat
  • Dizziness
  • Pale skin
  • Difficulty concentrating (“brain fog”)
  • Reduced exercise tolerance

Conventional Treatments for Chemotherapy-Induced Anemia

1. Determine the Cause First

Not every patient becomes anemic for the same reason.

Your oncology team should evaluate:

  • Complete blood count (CBC)
  • Iron studies
  • Ferritin
  • Vitamin B12
  • Folate
  • Kidney function
  • Evidence of bleeding
  • Bone marrow involvement

Treating the underlying cause often improves anemia significantly.


2. Blood Transfusions

Red blood cell transfusions provide the fastest improvement.

Advantages:

  • Rapid symptom relief
  • Improved oxygen delivery
  • Helpful for severe anemia

Disadvantages:

  • Temporary benefit
  • Requires a hospital or infusion center
  • Rare transfusion reactions
  • Iron overload with repeated transfusions

Transfusions are commonly used when hemoglobin becomes very low or symptoms become severe.


3. Erythropoiesis-Stimulating Agents (ESAs)

Examples include:

  • Epoetin alfa
  • Darbepoetin alfa

These medications stimulate the bone marrow to produce more red blood cells.

Current ASCO/ASH guidelines recommend considering ESAs primarily for patients receiving palliative (non-curative) chemotherapy whose hemoglobin has fallen below 10 g/dL. Because ESAs increase the risk of blood clots and may affect outcomes in some cancers, they are generally not recommended when chemotherapy is being given with curative intent.


4. Iron Replacement

Many patients receiving chemotherapy have functional iron deficiency.

Iron supplementation—particularly intravenous iron in selected patients—may improve the response to ESAs and reduce the need for blood transfusions. Iron status should be monitored before and during treatment.


5. Treat Vitamin Deficiencies

Correcting deficiencies of:

  • Vitamin B12
  • Folate
  • Iron

may improve anemia when laboratory testing identifies them.


Evidence-Based Complementary Therapies

These therapies generally support recovery rather than replace conventional treatment.


1. Optimize Protein Intake

Bone marrow cannot manufacture red blood cells without adequate amino acids.

Many cancer patients consume insufficient protein because of:

  • Nausea
  • Taste changes
  • Mouth sores
  • Appetite loss

Aim for approximately:

1.2–1.5 g protein/kg/day

(unless your physician recommends otherwise because of kidney disease.)

Good choices include:

  • Fish
  • Eggs

2. Iron-Rich Foods

Even when supplementation is not necessary, eating iron-rich foods supports healthy blood production.

Examples:

  • Pumpkin seeds
  • Oysters
  • Sardines

Pair plant-based iron with vitamin C-containing foods to improve absorption.


3. Vitamin C

Vitamin C enhances the absorption of non-heme iron from foods.

Good sources include:

  • Citrus
  • Strawberries
  • Bell peppers
  • Broccoli
  • Kiwi

Patients receiving certain chemotherapy drugs should discuss high-dose vitamin C supplements with their oncologist.


4. Moderate Exercise

It sounds counterintuitive.

Patients often believe they should rest when fatigued.

Research consistently shows that appropriately prescribed exercise can:

  • Reduce fatigue
  • Improve cardiovascular conditioning
  • Improve the quality of life
  • Maintain muscle mass

Even:

  • Walking
  • Stationary cycling
  • Resistance bands

may help.


5. Anti-Inflammatory Diet

Inflammation suppresses normal red blood cell production.

An anti-inflammatory diet emphasizes:

  • Fruits
  • Vegetables
  • Whole grains
  • Olive oil
  • Nuts
  • Fatty fish
  • Legumes

while limiting:

  • Ultra-processed foods
  • Refined sugar
  • Excess alcohol

6. Sleep Optimization

Bone marrow recovery occurs continuously, but poor sleep increases inflammatory cytokines that may worsen fatigue.

Helpful habits include:

  • Consistent sleep schedule
  • Morning sunlight
  • Limiting evening screens
  • Treating sleep apnea if present

7. Treat Other Causes of Fatigue

Many patients blame everything on anemia.

Other contributors include:

  • Thyroid dysfunction
  • Depression
  • Chronic pain
  • Infection
  • Poor nutrition
  • Medication side effects
  • Dehydration

Addressing these often improves energy even if hemoglobin changes little.


Therapies With Limited or Emerging Evidence

Patients often ask about supplements marketed for “building blood.”

Current evidence remains limited for chemotherapy-induced anemia.

Examples include:

  • Spirulina
  • Medicinal mushrooms
  • Chlorophyll supplements
  • Beetroot
  • Ashwagandha
  • Adaptogenic herbs

Some small studies suggest potential benefits, but none should replace evaluation for iron deficiency, transfusion needs, or ESA therapy. Because some supplements may interact with chemotherapy or affect clotting, discuss them with your oncology team before use.
